FGHA Volunteer 2023-24 Letter (when requesting VSS) (All volunteers are required to provide a valid VSS - Vulnerable Sector Screening. This form is required with the VSS application.)- OWHA CODA (Criminal Offence Declaration) Form (required annually the year after the VSS was issued)
